There are five sections to this registration form:
- Information about the person attending
- Medical information for the person attending
- Course selection
- Information about the parent/guardian
- Parental consent
Please be ready to complete all sections before starting the application.
After you submit the registration form there are several options for payment:
- Your deposit of $400 is due by July 1, 2024 with the balance of $210 due at registration on July 21, 2024
Payment can be made via PayPal or check (please see brochure for address). You will be directed to a PayPal page to pay the registration fee (in part or in whole).
